VALORACIÓN DEL ESPESOR ÍNTIMO-MEDIO DE LA CARÓTIDA COMO HERRAMIENTA PREVENTIVA EN ADULTOS BRASILEÑOS NO DIAGNOSTICADOS CON NIVELES DE TRIGLICÉRIDOS ALTERADOS

Resumen













La relación entre el perfil lipídico, el espesor íntimo-medio de la carótida (c-IMT) y la aterosclerosis subclínica no está completamente esclarecida. Actualmente, la medición de la c-IMT no se recomienda como prueba de detección de la aterosclerosis y las enfermedades cardiovasculares. Se realizó un estudio transversal que abordó la asociación entre el perfil lipídico y el engrosamiento de la carótida en adultos no diagnosticados. Se utilizó un análisis jerárquico para examinar la asociación entre el perfil lipídico y la c-IMT en cada sexo. El estudio incluyó a 201 adultos sin diagnóstico previo de enfermedad aterosclerótica o diabetes tipo 2 (edad media de 51 ± 14.6 años; 62.2% mujeres), seleccionados mediante muestreo aleatorio simple. La medida de resultado fue la c-IMT. Las variables independientes fueron los niveles de lipoproteína de baja densidad, colesterol total, triglicéridos, lipoproteínas de muy baja densidad y lipoproteínas de alta densidad. Las posibles variables de confusión fueron la edad, el ingreso familiar, la actividad física, el tabaquismo, la hipertensión, la obesidad y la diabetes. La c-IMT fue mayor (0.92 ± 0.33 μm) cuando los individuos presentaban valores alterados de triglicéridos [hombres: 0.001 g/L (IC 95%: 0.0002–0.002); y mujeres: 0.0005 g/L (IC 95%: 0.00009–0.001)] en el análisis ajustado. Esto sugiere que los niveles de triglicéridos son un marcador simple de aumento de la c-IMT en adultos brasileños no diagnosticados. En la práctica clínica, se alienta a los profesionales de la salud a medir tempranamente el espesor íntimo-medio de la carótida en pacientes asintomáticos con niveles elevados de triglicéridos, con el fin de prevenir la aterosclerosis y las enfermedades cardiovasculares.
