SHOCK INDEX COMO PREVISOR DE GRAVIDADE NO CHOQUE HEMORRÁGICO: UMA REVISÃO CRÍTICA

Resumen
Este estudo visa revisar criticamente a literatura existente sobre o uso do Índice de Choque (SI) como previsor de gravidade em pacientes com choque hemorrágico, avaliando sua eficácia e limitações em comparação a outros indicadores clínicos. Realizou-se uma Revisão Integrativa da Literatura, seguindo as diretrizes do Joanna Briggs Institute (JBI). A pesquisa foi conduzida em bases de dados eletrônicas, incluindo PubMed, SciELO e LILACS, utilizando termos como “Shock Index” e “Hemorrhagic Shock”. Foram incluídos estudos publicados entre 2019 e 2024 que analisaram o IS em contextos de trauma e cirurgia. A qualidade dos estudos foi avaliada por dois revisores independentes, com divergências resolvidas por consenso. Os resultados mostraram que o IS é amplamente eficaz na triagem de pacientes em risco de complicações graves, como a necessidade de transfusões e internações em UTI. Contudo, o IS apresenta limitações em subgrupos específicos, como pacientes pediátricos e obstétricos, para os quais ajustes do índice demonstraram maior precisão preditiva. A análise também destacou a utilidade do IS em combinação com outras ferramentas diagnósticas. O Índice de Choque é uma ferramenta valiosa na prática clínica, especialmente em emergências. No entanto, adaptações, como o IS ajustado para idade, são necessárias para melhorar sua eficácia em diferentes populações. Pesquisas futuras devem explorar essas adaptações e o uso de novas tecnologias.
