SHOCK INDEX AS A PREDICTOR OF SEVERITY IN HEMORRHAGIC SHOCK: A CRITICAL REVIEW

Abstract

This study aims to critically review the existing literature on the use of the Shock Index (SI) as a predictor of severity in hemorrhagic shock patients, assessing its effectiveness and limitations compared to other clinical indicators. An Integrative Literature Review was conducted, following Joanna Briggs Institute (JBI) guidelines. The research was performed in electronic databases, including PubMed, SciELO, and LILACS, using terms like "Shock Index" and "Hemorrhagic Shock." Studies published between 2019 and 2024 that analyzed the SI in trauma and surgery contexts were included. The quality of the studies was evaluated by two independent reviewers, with disagreements resolved by consensus. The results demonstrated that the SI is widely effective in screening patients at risk of severe complications, such as the need for transfusions and ICU admission. However, the SI shows limitations in specific subgroups, such as pediatric and obstetric patients, where index adjustments proved more accurate. The analysis also highlighted the usefulness of the SI in combination with other diagnostic tools. The Shock Index is a valuable tool in clinical practice, particularly in emergencies. However, adjustments, such as age-adjusted SI, are necessary to improve its effectiveness in different populations. Future research should explore these adaptations and the use of new technologies.
